Comparative Analysis of Cemented Versus Cementless Total Knee Replacement Arthroplasty



Abstract
Cementless total knee arthroplasty was developed recently to overcome the projected durability of methylmethacrylate fixation. But many authors had reported remarkably good results with a number of different cemented devices.
Between january 1987 and December 1991, 61 total knee replacement arthroplasty were performed with cemented prosthesis and 22 total knee replacement arthroplasty with cementless prosthesis. The average follow-up period was 31 months and 23 months
respectively. Using the knee rating scale of the Hospital for Special Surgery and the American Knee Society Roentgenographic Evaluation and Scoring System, clinical and radiological comparative study between cemented and cementless group was done
and
the results were as follows;
1. Pain scores improved from 11.9 to 26.2 in cemente group (group 1) and from 11.4 to 27.5 in cementless group(group II). and there was no significant difference between the two groups(p>0.05).
2. ROM of the kness improved from 88¡Æto 94¡Æin group I and from 97¡Æto 108¡Æin group II(I>0.05).
3. Knee scores improved from 48 to 83 in group I and from 59 to 91 in group II(p>0.05).
4. Tibiofemoral angles were corrected from varus 9.5¡Æto valgus 5.7¡Æin group I and from varus 13.4¡Æto valgus 5.0¡Æin group II(p>0.05).
5. There was no significant differences in component position and scores of radiolucent lines between the two groups(p>0.05). But, in both groups, the scores of radiolucent lines were singificantly higher in zone 1 & 4 of A-P roentgenograph of
the
tibial components than in other zones(p>0.05).
In conclusion, there were no significant differences in clinical and radiological results betwen the cemented and the cementless groups. But the factors of disease pattern, types of implants and follow-up periods should be considered to do a
standardized paired analysis.
